1. Membership Declaration
I/we declare that I/we are over 18 years of age (and for members who are under 18, I/we the parent(s)/legal guardian(s) of my child/ren, who is/are under 18 years old). I/we, declare that by performing, me/my child/ren are a voluntary member/s of The Ozy Youth Choir Honouring Defence Service, Inc. (referred to as “the Association”).
I/we agree that my child and our family will follow the Choir’s Code of Conduct, which includes:
Supporting a positive, team-oriented environmentActing with honesty and integrity
Following directions from MOB Leaders, official helpers, and any designated Choir leaders during practices and performances
Treating everyone with respect, courtesy, and without harassment
Following all reasonable and lawful instructions from MOB Leaders, official helpers, or other Choir leaders
Using the Association’s resources responsibly and only for Choir-related purposes
Accessing the Association’s website (including the Calendar) only as a Choir member
I/we understand that any breach of the Code of Conduct by myself, my child, or our family members may lead to immediate termination of our membership, with no membership rights retained.
2. Talent Release Statement
I/we give permission to The Ozy Youth Choir Honouring Defence Service, Inc., its agents, and legal representatives to use any still and/or moving images (including video, photographs, audio recordings, or social media content) of my/our child taken at Choir events, as needed by the Association, for:
Advertisements, marketing, and official recordingsEducational, training, or publicity purposes promoting the Association’s activities
I/we understand that this consent applies worldwide and remains in effect indefinitely.
I/we confirm that I/we have the legal authority to provide this consent on behalf of my/our child. I/we acknowledge that any images or recordings taken by the Association or its representatives may be used commercially to promote the Association’s activities.
By allowing my child to participate in Choir events and performances, I/we agree to all terms and conditions stated above.
3. MOB Leaders and Official Helpers Code of Conduct
All MOB Leaders and official helpers commit to:
Demonstrating professionalism in their roleActing with honesty, integrity, and diligence
Treating everyone with respect, courtesy, and without harassment
Avoiding any conflicts of interest related to their position
Following all policies and protocols, including child protection policies
